Requesting Accommodations
STEP 1: Gather Appropriate Documentation
For specific guidelines on documentation, refer to the Documentation Guidelines below,
It is possible that you will be asked to provide additional documentation or that a consultation with an evaluator will be necessary before a decision can be made.
学生将收到残疾服务办公室的信息, through their SU email or a text message, to schedule a meeting with a Disability Services staff member once your request for 住宿 and documentation have been reviewed.
Documentation Guidelines
Susquehanna University students who seek 住宿 based on their disability are responsible for completing an online Disability Disclosure and Request for Accommodation form and for providing appropriate supporting documentation. A student who seeks academic 住宿 on the basis of a disability is responsible for obtaining the needed evaluation and resulting documentation, 并主动与残疾服务办公室联系. Susquehanna University approaches requests for 住宿 with the belief that each student’s circumstance is unique and that a flexible approach is the right way to determine appropriate 住宿.
本身, a 医疗 diagnosis does not establish a need or an entitlement to 住宿 under Section 504 of the Rehabilitation Act or the Americans with Disabilities Act as Amended (ADA-AA). 因此, the documentation required by Susquehanna University extends beyond the 医疗 diagnosis and encompasses the four key elements of a person’s disability status under Section 504 和 ADA. Those key elements are that:
the degree of limitation is substantial, and
Documentation that substantiates each of these four areas serves as a foundation that legitimizes a student’s request for appropriate 住宿. The Director or Assistant Director of Disability Services will review the documentation and collaborate with the student to determine reasonable and appropriate academic 住宿. The 住宿 afforded to each student will be determined on an individual basis and will be based on the documentation provided.
Accompanying documentation such as IEPs or 504 plans or letters from previous educational institution’s disability office are welcomed to complement 医疗 documentation; however, these supporting materials alone 不 提供住宿需求的证明文件,不作为住宿申请的批准文件.
一份高中计划对于决定适合的高中生活非常有帮助, but these plans 不 always include specific recommendations for 住宿 that can be implemented in the college setting or an indication of how students’ needs may have changed over time. 学生s who are eligible for 住宿 in high school are encouraged to request an updated assessment prior to graduation from high school. Check out the difference between高中vs.
*The Medical Provider Form, According to the ADA, 一个主要的生命活动也包括一个主要身体功能的运作, 包括, but not limited to, functions of the immune system, normal cell growth, 消化, 肠, 膀胱, 神经系统, 大脑, 呼吸, 循环, endocrine and reproductive functions.”

旧的信息, while providing a history of impairment, 是否不能充分解决当前致残状况的影响.
IEPs and 504 Plans, early psychoeducational evaluation reports, 老师的评论, 来自导师或学习专家的文档可能都是有用的信息来源,但是, in and of themselves, 是否不被视为临床文件,以提供住宿.
Checklists and/or surveys can serve to supplement the diagnostic profile but in and of themselves 不 adequate for the diagnosis of ADHD and 不 substitute for clinical observations and sound diagnostic judgment.
评估应包括针对注意力不集中的标准化措施, hyperactivity and impulsivity as well as diagnostic information to demonstrate functional impairment in an educational setting.
